The Request for Work Permit and Statement of Intent to Employ Minor can be downloaded below (MS Word format).  Please note: THIS IS NOT A WORK PERMIT.

The Student MUST return the completed form to the issuing authority in order to obtain a Work Permit. Student MUST be present to sign the Work Permit.
